RESUMO

AIM: to evaluate functional results of reoperation performed according to the CIRCLE technology and using the VisuMax femtosecond laser and MEL-80 excimer laser in cases of regression of the refractive effect after SMILE surgery. MATERIAL AND METHODS: We studied a group of post-SMILE patients. In those, who showed regression of the refractive effect at 1 year, reoperation was performed according to the CIRCLE technology and using the VisuMax femtosecond laser. The corneal flap was separated from the stromal bed and turned aside. Excimer laser ablation of the stromal bed was performed with the MEL 80 machine. The corneal flap was then placed back and rinsed from both sides. Uncorrected (UCVA) and corrected (BCVA) visual acuity as well as spherical equivalent (SE) were estimated before reoperation, on day 1, and at 1 month. RESULTS: After reoperation, BCVA and UCVA improved. Patient refraction became close to emmetropia. Specifically, UCVA was 0.23±0.18 at baseline (i.e. 1 year after SMILE) and 0.93±0.11 after the CIRCLE procedure (p<0.05). The absolute value of SE was 1.86±1.15 D and 0±0 D before and after CIRCLE, respectively (p<0.05). BCVA change was not statistically significant - from 0.95±0.1 to 0.93±0.11 (p>0.05). CONCLUSION: Reoperation performed according to the CIRCLE technology and using the VisuMax femtosecond laser and MEL-80 excimer laser provides an increase in visual acuity in case of post-SMILE regression of the refractive effect.

The effect of an original sulfated glycosaminoglycan (sGAG) mixture (keratan sulfate with chondroitin sulfate) was examined in the in vitro experiment with the cell culture of murine fibroblasts (cell line L929). Two forms (solution and gel) of the active substance were tested in different concentrations (0.1-5.0%). The growth dynamics of fibroblasts in culture was traced for the periods up to 144 h using the automated digital immersion confocal microscope. The study of the original sGAG mixture demonstrated the bimodal effect: low concentrations (0.1-0.5%) stimulated the proliferation of fibroblasts, while high concentrations (1-5%) possessed an antiproliferative effect. Changes of the form (solution or gel) had no effect on the functional properties of the mixture. High sGAG concentrations (3-5.0%), along with the antiproliferative effect, had a hidden weak cytotoxic action (a delayed effect).

RESUMO

A technique for dynamic refraction measurement in natural conditions is proposed using open-field refractometer. Dynamic refractometry was studied in 101 schoolchildren with emmetropia and different degree ametropia. Based on the data obtained accommodation input for 5, 2.5, 1, 0.5 and 0.33 m was countered. It was found that as the object becomes closer spheric power and astigmatism degree increase and axis changes. Accommodation input in patients with hyperopia was close to calculated values, in emmetropia decrease of input by mean 49% was found and in myopia--by 74% compared to calculated values.

Presence and pattern of peripheral retinal changes were studied using ultrasound biomicroscopy (UBM) in patients with cataract of different etiology. UBM was performed in 85 eyes of 65 patients. Mean age was 65, 75+8, 19 years old. UBM was performed using "Sonomed" equipment (USA). Morphology of revealed by means of UBM peripheral retinal changes was made in 5 eyes with uveal melanoma. The signs of retinoschisis were found in 82 eyes (96.5%) using UBM. The initial UBM signs represent inclusions in the inner retinal layers. With the progressing of schisis cystic splitting of retina was found. Confluence of distinct cysts into one schisis zone was characteristic for further progressing. Thus peripheral retinoschisis has acoustic stages, that are confirmed morphologicaly and can be revealed by means of UBM in the majority of old patients with cataract of different etiology.

RESUMO

The aim of this work was to solve the problem of prognostification of anatomic and functional results of treatment of retinal detachment (RD) with silicone oil tamponade. Analysis of the treatment of 257 eyes was performed using pattern recognition methods. All the patients were characterized according to 32 signs related the preoperative status only. As a result, an informative sign system was developed; the present data proved to be not representative enough, which manifested mostly in the class of RD recurrence, where the rate of correct end result recognition was 75% (in the class of "normal condition" it was 95%). One of the most significant RD signs, PVR degree, in this study was placed in the second ten according to its informative value, probably due to the fact that PVR classification is not detailed enough.

The article is dedicated to academician S. N. Fyodorov, an outstanding surgeon, scientist, and a public figure. The paper covers the milestones of S. N. Fyodorov's creative way. His motto was uncompromisingness, diligence, and innovation. The main result of academician Fyodorov's many-year work, his brainchild, was Eye Microsurgery Complex, an inter-branch scientific and technical establishment, which became the material base for his most daring projects. S. N. Fyodorov made a real revolution in ophthalmology by changing it from a modest surgeless science into a bright and stormily progressing field of medicine. Thanks to his achievement, Russia is still one of the world's leaders in ophthalmology. Today, Eye Microsurgery is the flag of Russian ophthalmology. It provides more than 50% of all high-technology ophthalmologic aid in the country.

RESUMO

We studied the mechanisms of action of imidazobenzimidazole derivative RU-1117 on calcium homeostasis in myocytes isolated from rat thoracic aorta. In therapeutic concentrations, RU-1117 increased the content of free calcium ions due to their mobilization from intracellular depot via the IP3-dependent mechanisms. Antagonists of angiotensin II AT1-receptors irbesartan and, to greater extent, eprosartan abolished the calcium-mobilizing action of RU-1117. Selective antagonist of endothelin ETA-receptors sitaxsentan and alpha1-adrenoceptor agonist prazosin produced no effect on calcium mobilization caused by novel local anesthetic RU-1117.

RESUMO

Conformation and kinetic characteristics of the interactions of local anesthetics lidocaine (xycaine), tetracaine (dicaine), bupivacaine, and new RU-1117 compound with proven anesthetic activity with Visiton (1% hydroxypropylmethylcellulose in phosphate buffer) were studied. It was found that complex formation between the local anesthetics and hydroxypropylmethylcellulose is a time-dependent reversible process. The equilibrium is attained within 2.5-8.0 h and depends on the chemical nature of local anesthetic.

RESUMO

Optic coherent tomography was used to monitor three cases of successful laser retinal coagulation in involutional macular degeneration followed by transsudative retinal detachment of the pigment epithelium and neuroepithelium to give rise to a subretinal neovascular membrane. The technique was shown to be of high informative value in providing an accurate localization of abnormal macular changes and in monitoring their state. The application of optic coherent tomography could reduce the number of control angiographies and, in a number of cases, to obviate a necessity of performing angiographic studies.

Wrong decision on timely elimination of an abnormality at the level of the lacrimal ducts, irrational surgical treatment policy, and postoperative management in patients with abnormal parameters of the lacrimal system lead to recurrent epiphora. The polymorphism of the causes underlying the development of an unstable lacrimal film requires that individual rehabilitative treatment policy should be chosen for victims.
